We only accept Aetna insurance or private pay.

For patients with Aetna Insurance or Florida Medicare:

Please complete the contact information and you will receive a 15-minute phone call to discuss how we can best help you or your child.   Once the call is completed in order to schedule you will receive 2 emails; one from Headway and one from Osmind/Priority Mental Health.  Please complete all the paperwork that is sent and set up your Headway account. 

Once your paperwork is completed and Headway verifies that we accept your plan, you will be scheduled and receive a zoom link.  Headway will determine the cost of your co-pay.  You will be asked to leave a credit or debit card on file for Headway and Priority Mental Health.  Once all the paperwork is complete with Osmind and Headway, you will be contacted to make an initial appointment. For patients with Florida Medicare:

Please complete the contact information and you will receive a 15-minute phone call to discuss how we can best help you or your child.   Once the call is completed in order to schedule you will receive an email from Osmind/Priority Mental Health.  Please complete all the paperwork that is sent and you will be contacted to make an initial appointment.   Thank you.

For private pay patients:

Payment is due at the time of service.  Upon request, a superbill may be provided to you so that you may submit it to your insurance company on your own behalf. Your insurance company may reimburse you according to their out-of-network reimbursement policies, which may vary widely depending on the company. Call your insurance company to check your out of network policy.

Please provide your full name, child’s name, if applicable, patient date of birth, email, address, a brief summary of how we may be of assistance to you or your child. In addition, please upload copy of the front and back of your Aetna card or Florida Medicare card, if applicable. Once we receive your information you will receive a packet of information to complete. We will contact you within 24-48 hours.
